Online Assessment Form Activity: Creating Observation Forms

Observation Forms are part of the practical assessment that guides the assessor in grading the candidate’s task. To make it easier for the assessor during the assessment attempt of the student, the Observation Form can be pre-loaded into the activity

Follow the steps below on how to create an Observation Form:

Note: You need a Trainer or Manager access to perform this action

Creating an Observation Form


Step 1: Select  the course and activity you wish to add your observation form to


Step 2: Click the Observation Form tab


Step 3: Click Create Observation Form

Step 4: Input the Observation Form title. You may also add a summary or description of the Observation Form. Then click Save changes:

Step 5: Click the Manage Observation Points button beside the Observation Form title:


Note:
The icons indicate the following:
 Manage Observation Points - This is used to add and edit observation points.
 Edit Observation Form - This allows the user to edit the Observation Form title and description.
 Duplicate - This allows the user to duplicate the Observation Form including the points.
 Delete - This is used to delete the observation form

Step 6: To add a main point, click Create Observation Point

On the space provided, type in the observation item then click save:


Step 7: To add another main point, click again Create Observation Point. To add a subpoint, click the  beside the initial observation point. Add the criteria in the space provided. Then click save changes.



A subpoint is indented to the left under the main point.



Notes:
Clicking the  beside the observation point will create subpoints for that grade item. These pointes will be indented towards the right. To add another main point, click Create Observation Form



Step 8: Add all the necessary observation points. Once all the points have been added, click Back to Observation Forms


Adding the Observation Form to the Task Field

Step 1: To add the observation form to the task. click Edit Assessment. Then click Update Assessment.



Step 2: Select the tab where you need to add the Observation Form to. 



Step 3: In the task field area, click the blue Observation Form button. In the right panel under the Input tab, click the dropdown list under Observation Form. Choose the Observation form you need to use for the task. You may also change the text title to reflect the name of the observation form.



Step 4: If you have more tasks that need observation forms, repeat the previous steps. Then Click save form to save your changes.



Note: You may only use the observation form per task field. Should there be multiple tasks requiring the same observation form, duplicate the form and add the copies in the succeeding tasks.

You have now added an Observation form to your task.

Assessor View of the Observation Form

Once used in the task, a new window will appear which allows the assessor to complete the observation form while checking the task at the same time.
