Online Assessment Form Activity: Creating Multiple Choice Questions

Multiple choice questions can be built in the Online Learning Assessment Form. These types of questions are manually graded by the assessor.

Follow the steps below on how to build multiple choice questions:

Note: You need a Trainer or Manager access to perform this action

Multiple Choice Questions with One Correct Answer

Step 1: In the design mode of the OLA form, select the Radio Button field and drag it to the design area:



Step 2: Click the blue pencil icon to edit its properties.




Step 3: You may add your question using the label tab. 



*If your question is longer than 1 sentence, you may opt to use the HTML View field (for paragraphs) or Headings 5.

Step 4: Under the Input tab, click add new option and type in your options in the space provided




Step 5: Add the correct answer in the marking guide tab



Step 6: Add a task field. Insert the Radio button field inside the task field. You may add more elements in the task as needed.

Kindly remember though that 1 task field = 1 point. 
If you add more another multiple choice question inside the same task field, it will be counted as part of the previous question

Step 7: Click save form to save your changes

Step 1: In the design mode of the OLA form, select the Checkbox field and drag it to the design are:


Step 2: Click the blue pencil icon to edit its properties.



Step 3: You may add your question using the label tab. 

*If your question is longer than 1 sentence, you may opt to use the HTML View field (for paragraphs) or Headings 5.


Step 4: Under the Input tab, click add new option and type in your options in the space provided


Step 5: Add the correct answer in the marking guide tab


Step 6: Add a task field. Insert the Radio button field inside the task field. You may add more elements in the task as needed.
